Allergic conditions in children with bronchial asthma and their relation with age and peripheral blood eosinophil counts

目的 基于血清特异性IgE(sIgE)水平,分析支气管哮喘(简称哮喘)患儿的过敏情况及其与年龄、外周血嗜酸性粒细胞(EOS)计数的关系。方法 回顾性分析120例哮喘患儿的临床资料,根据年龄将患儿分为A组(1岁<年龄≤3岁)15例、B组(3岁<年龄≤6岁)56例、C组(6岁<年龄≤15岁)49例。分析哮喘患儿血清总IgE阳性情况、吸入性及食入性过敏原sIgE阳性情况。比较3组患儿血清总IgE水平、吸入性及食入性过敏原sIgE水平。分析屋尘螨/粉尘螨sIgE阳性患儿的过敏程度与年龄、外周血EOS计数的关系。结果 (1)120例哮喘患儿中,有110例(91.67%)患儿血清总IgE阳性;A组、B组、C组的血清总IgE阳性率分别为73.33% (11/15)、91.07% (51/56)、97.96%(48/49),C组的血清总IgE阳性率高于A组(P<0.05)。(2)120例哮喘儿童中,屋尘螨/粉尘螨sIgE阳性例数最多;A组哮喘患儿以屋尘螨/粉尘螨、霉菌混合、牛奶过敏为主,B组哮喘患儿以屋尘螨/粉尘螨、鸡蛋、大豆过敏为主,C组哮喘患儿以屋尘螨/粉尘螨、蟑螂、狗毛过敏为主。C组哮喘患儿屋尘螨/粉尘螨sIgE阳性率高于A组和B组,而A组哮喘患儿霉菌混合sIgE阳性率高于C组(P<0.05)。(3)67.06%(57/85)的屋尘螨/粉尘螨sIgE阳性哮喘患儿为特重度过敏。屋尘螨/粉尘螨sIgE阳性哮喘患儿的过敏程度与外周血EOS计数呈正相关,且C组患儿的屋尘螨/粉尘螨过敏程度较A组与B组严重(P<0.05)。结论 哮喘患儿的血清总IgE水平升高,且随年龄增加总体呈现升高趋势。屋尘螨/粉尘螨是哮喘患儿的主要过敏原,且随着年龄增加,哮喘患儿的常见过敏原由混合性过敏原逐渐过渡为吸入性过敏原。屋尘螨/粉尘螨过敏的哮喘患儿的过敏程度随着年龄增加而加重,且与外周血EOS计数相关;外周血EOS计数可作为评估屋尘螨/粉尘螨过敏的哮喘患儿过敏程度的参考指标。
ObjectiveTo analyze the allergic conditions in children with bronchial asthma (hereinafter referred to as asthma), and their relation with age and peripheral blood eosinophil (EOS) counts based on serum specific IgE (sIgE) level. MethodsThe clinical data of 120 children with asthma were retrospectively analyzed, and they were divided into group A (1 year old<age≤3 years old, 15 cases), group B (3 years old<age≤6 years old, 56 cases), or group C (6 years old<age≤15 years old, 49 cases) according to children′s age. Serum total IgE positive states, sIgE positive states of inhaled and ingestive allergen in children with asthma were analyzed. Serum total IgE level, inhaled and ingestive allergen sIgE levels were compared between children of the 3 groups. The relation of allergy degree of children with Dermatophagoides pteronyssinus/Dermatophagoides farinae sIgE positive with age and peripheral blood EOS counts was analyzed. Results(1) Among 120 children with asthma, serum total IgE of 110 (91.67%) children was positive; in addition, the positive rates of serum total IgE in groups A, B, and C were 73.33% (11/15), 91.07% (51/56), and 97.96% (48/49), respectively, and group C exhibited a higher positive rate of serum total IgE as compared with group A (P<0.05). (2) Among 120 children with asthma, the case number of Dermatophagoides pteronyssinus/Dermatophagoides farinae sIgE positive was the most. Children with asthma in group A were mainly allergic to Dermatophagoides pteronyssinus/Dermatophagoides farinae, fungal combination, and milk, asthmatic children in group B were mainly allergic to Dermatophagoides pteronyssinus/Dermatophagoides farinae, egg, and soy, and asthmatic children in group C were mainly allergic to Dermatophagoides pteronyssinus/Dermatophagoides farinae, cockroach, and dog hairs. The positive rate of Dermatophagoides pteronyssinus/Dermatophagoides farinae sIgE of asthmatic children in group C was higher than that in groups A and B, while the positive rate of fungal combination sIgE of children with asthma in group A was higher than that in group C (P<0.05). (3) A total of 67.06% (57/85) asthma children with Dermatophagoides pteronyssinus/Dermatophagoides farinae sIgE positive were extraordinarily severe allergy. In asthmatic children with Dermatophagoides pteronyssinus/Dermatophagoides farinae sIgE positive, allergy degree positively correlated with peripheral blood EOS counts, and allergy degree of Dermatophagoides pteronyssinus/Dermatophagoides farinae was more severe in group C than in groups A and B (P<0.05). ConclusionSerum total IgE level is elevated in asthmatic children, and with the increase of age, the general trend is increasing. Dermatophagoides pteronyssinus/Dermatophagoides farinae are leading allergens for children with asthma, and the common allergens in asthmatic children gradually transition from mixed allergens to inhaled allergens with the increase of age. Allergy degree in asthmatic children with Dermatophagoides pteronyssinus/Dermatophagoides farinae allergy increases with age and relates to peripheral blood EOS counts. Peripheral blood EOS counts can be used as a reference index to evaluate allergy degree in asthmatic children with Dermatophagoides pteronyssinus/Dermatophagoides farinae allergy.
