目的探讨当归续骨汤加减结合手法复位联合新型夹板外固定治疗桡骨远端骨折患者的临床效果。方法将98例桡骨远端骨折患者随机分为观察组和对照组,每组49例。对照组采用手法复位联合新型夹板外固定治疗,观察组采用当归续骨汤加减结合手法复位联合新型夹板外固定治疗。比较两组患者的疗效,治疗前后的疼痛介质(P物质、β-内啡肽、血清前列腺素E2、神经肽Y)、Gartland-Werley腕关节评分及骨代谢指标[总Ⅰ型前胶原氨基末端肽(P1NP)、Ⅰ型胶原羧基端肽β(β-CTX)、核因子 κB受体活化因子(RANK)、核因子 κB受体活化因子配体(RANKL)、抗酒石酸酸性磷酸酶5b(TRACP5b)]。结果观察组总有效率高于对照组(P<0.05)。治疗后,两组的血清P物质、β-内啡肽、前列腺素E2、神经肽Y、P1NP、β-CTX水平及Gartland-Werley腕关节评分均较治疗前降低,血清RANK、RANKL、TRACP5b水平均较治疗前升高,且观察组上述指标均低于对照组(均P<0.05)。结论当归续骨汤加减结合手法复位联合新型夹板外固定治疗桡骨远端骨折患者的效果显著,可抑制破骨细胞功能,减轻疼痛,改善腕关节功能,疗效优于手法复位联合新型夹板外固定治疗。
ObjectiveTo explore the clinical effect of modified Danggui Xugu Decoction combined with manual reduction and novel splint external fixation for treating patients with distal radius fracture. MethodsA total of 98 patients with distal radius fracture were randomly divided into observation group or control group, with 49 cases in each group. The control group received manual reduction combined with novel splint external fixation for treatment, and the observation group was treated with modified Danggui Xugu Decoction combined with manual reduction and novel splint external fixation. The efficacy, pre- and post-treatment pain mediators (substance P, β-endorphin, serum prostaglandin E2, neuropeptide Y), Gartland-Werley wrist joint score, bone metabolism indices (procollagen type Ⅰ N-terminal propeptide[P1NP], C-terminal telopeptide of type Ⅰ collagen [β-CTX], receptor activator of nuclear factor kappa B [RANK], receptor activator of nuclear factor kappa B ligand [RANKL], and tartrate-resistant acid phosphatase 5b [TRACP5b]) were compared between patients of the two groups. ResultsThe total effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, the levels of serum substance P, β-endorphin, prostaglandin E2, neuropeptide Y, P1NP, β-CTX, and Gartland-Werley wrist joint score were decreased as compared with before treatment, whereas levels of serum RANK, RANKL, and TRACP5b were elevated as compared with before treatment, and the observation group yielded decreased aforesaid indices compared with the control group (all P<0.05). ConclusionModified Danggui Xugu Decoction combined with manual reduction and novel splint external fixation for the treatment of patients with distal radius fracture exerts a significant effect, which can inhibit osteoclast function, relieve pain, and ameliorate wrist joint function, and its efficacy is superior to manual reduction combined with novel splint external fixation therapy.
